We specialize in Respiratory Services that cannot be beat! We offer all of the following to stay with the Respiratory patient from hospital back to home:
- Readmissions Solutions Program-This program focuses on admitting, treating, maintaining and spotlighting the RT patient with the whole goal of NOT sending the patient back to the hospital unnecessarily. The typical RT patient in the nursing home world goes back to the hospital 15-20% of the time. Our program runs less than 2%.
- Respiratory Telehealth– Get a live RT in front of your patient within minutes. This service can be done in a patients home, nursing facility, or anywhere they may be.
- Home Readmissions Reduction Prevention Program-Intensive pre discharge education with 5 follow up phone calls/telehealth to ensure that the patient is progressing in the right way after their rehab. Incredibly successful with stopping patients for going back to the hospital unnecessarily.
- RT Capture Program (Increase PDPM rates up to $160.00 per day per patient)
- Remote Monitoring Program-We monitor all new admits and long term patient population to determine what is the best course of action for the compromised respiratory patient.
- 24 Hour Respiratory visits and on call-We provide round the clock patient visits to stop patients from going back to the hospital unnecessarily.
